What are potential symptoms of low T?
Hot flashes (when it is severe and/or a sudden drop)
Diminishing sexual hair, muscle mass, and bone mineral density if a profound deficiency is present and low T levels have been neglected to a significant degree for several years

If it is determined I have low-suboptimal T levels, how will you help?
5. Ongoing Monitoring: Regular follow-up appointments will ensure treatment is effective and adjusted as necessary. (Don’t forget sick visits are included for you as a courtesy.)
6. Educative Resources: The center will offer information sessions and materials on understanding low testosterone and its effects.
8. Additional Health Screenings: The center may conduct screenings for related health issues like diabetes and cardiovascular health.
9. Access to Specialists: For moderate to severe cases, referral to an endocrinologist and other specialists for comprehensive care may be warranted.
Most Google information has not been updated since the new FDA release of February 2025. Current FDA TRAVERSE trials from 2023 concluded that there was no increase in the risks of adverse cardiovascular outcomes in men using TRT for hypogonadism; however, they did confirm an increase in blood pressure. This risk is more prominent with oral T undecanoate or subcutaneous T enanthate. Our skilled practitioner understands these risks, so we have developed the systems we have in place.
Additionally, T undecanoate IM carries a risk of Pulmonary oil microembolism.
Topical gels and transdermal solutions carry a risk of secondary exposure, so it is increasingly important to understand their safety.
Hepatotoxicity has been limited to oral T formulations. However, we monitor and observe this risk for all formulations.
Our standard go-to T formulation is cypionate injectable due to the additional side effects of other formulations.
So let’s talk about it.
Blood pressure increases are potentially secondary to fluid retention and peripheral edema. Cypionate is contraindicated with those with liver impairment due to this fluid retention. Our plan: Monitor blood pressure and peripheral edema, complete a metabolic panel, and adjust treatment as indicated.
Polycythemia/erythrocytosis, exact mechanism unknown, hypothetical thoughts include the role of hepcidin, iron sequestration and turnover, erythropoietin production, bone marrow stimulation, and genetic factors. Our plan: monitor hematocrit and hemoglobin levels and adjust treatment as indicated, as well as therapeutic phlebotomy with HCT levels equal to or above 54%
Prostate cancer and benign prostatic hypertrophy risks. Evidence is conflicting. With BPH, if you have severe AUA/International Prostate symptom score >19 and lower urinary tract symptoms, unfortunately, you are excluded from being able to enroll in the TRT program. If you have known prostate cancer, you will need a urology clearance before proceeding. Our plan: We monitor your PSA levels closely and refer to urology if necessary. It is important to note, PSA levels do tend to rise directly after initiation of TRT, however, this is most often transient.
Worsening of sleep apnea. Our plan: If you have untreated sleep apnea, or suspect you have undiagnosed sleep apnea, we will refer you back to your PCP for sleep study and treatment.
Infertility and testicular atrophy. Our plan: We recommend that our men also utilize HCG or Gonadorelin to help decrease the effects of testicular atrophy and infertility, which are undesirable and expected side effects of TRT. Gonadorelin may help with testicular atrophy, but is inferior to HCG for fertility. HCG due to FDA regulations is quite expensive, unfortunately. You can expect to add an additional $150 a month to your plan if you prefer HCG to Gonadorelin. We encourage men who want to start a family within the next few years to try formulations and/or natural methods of increasing their T levels, such as Clomid, HCG, diet, exercise, weight management, etc.

I often get men asking if their natural T-making abilities will decrease if they start on TRT. It is imperative to note that TRT is considered a lifelong treatment. Once you start and reap the benefits, you will have to continuously take the medication to keep your T levels up, as the treatment does not work only once to increase your levels and then sustain them. There is no definitive proof that your body stops producing T to the point it can’t recover after ceasing TRT. Research indicates the longer you are on TRT, the longer it may take for your body to return to “your” normal levels, and sudden drops can cause mood changes and hormone fluctuations. Our plan: If you decide to stop TRT, we will do what we call a weaning protocol to help mitigate the withdrawal effects of coming off of TRT.
It is crucial to understand these potential effects and decide what is right for you. For men with significant symptoms, TRT can be a life-changing intervention that can be utilized throughout their lives.
What kind of long term things should I consider if I want to try TRT?
What type of associated costs should I expect with the urgent care visits and STD screening visits?
Your visit to our nurse practitioner for a basic sick visit or STD screening visit, is fully covered under your monthly membership fee, ensuring that you receive essential healthcare without additional stress. These can be conducted via telehealth or in the office. However, please note that extra charges may apply for lab testing, medications, or in-office procedures. Examples are: what you pay the pharmacy if an antibiotic is called in, or the STD panel testing, or if you would benefit from an in-office dexamethasone injection, etc.

During your office visit, we’ll discuss your symptoms and current efforts to address them. You will then be offered an initial treatment plan and your labs drawn for further testing. This testing will confirm whether you are eligible to receive hormone replacement therapy and join the TRT membership program.
Note: your initial lab tests will include a lipid panel for which you must fast. We suggest coming a couple of days before your appointment to the office to complete the lab work. It makes your initial appointment much more informative with your actual lab values instead of speaking hypothetical numbers.
If you are already undergoing hormone replacement therapy (TRT) or other medications, and are considering switching to our services, we will review your current treatment plan and determine if you are due for any necessary lab testing. You should bring a physical copy of your last lab results to your appointment to avoid repeating unnecessary labs.
Your initial office visit is charged at $300 to help offset the extensive comprehensive lab panel costs. We value you as a patient and want to evaluate your metabolic status comprehensively. If your lab results and symptoms indicate a need for hormone replacement therapy and you choose to become a patient, you can enroll in the membership.
Your $300 is required before any service. For example, if you come in for lab testing before your first appointment (which is encouraged) you will be responsible for paying the costs at that time.
Once you enroll, your first membership payment will not be processed until the next scheduled payment date. For example, our payment installments are scheduled for the 27th of each month, and hypothetically, your first visit is on the 14th; you will not be required to make another payment on the 27th. Instead, your first membership payment would be due the following month.
